Polymyalgia Rheumatica: Managing Morning Stiffness, Fatigue, and Inflammation Naturally

You wake up and try to lift your arms above your head. It shouldn't be hard. But it is. Your shoulders feel locked. Your hips ache. It takes a solid hour — sometimes more — before your body feels like it belongs to you again.

This is polymyalgia rheumatica. And if you've been recently diagnosed, you're probably still absorbing the reality of it.

PMR is a common inflammatory condition — more common than many people realize — and yet it often goes undiagnosed for months because the symptoms can look like "just getting older." It's not. It's a real autoimmune inflammatory condition, and it deserves real attention.


What Makes PMR Distinct

Polymyalgia rheumatica (PMR) is an inflammatory condition characterized by pain and stiffness in the shoulder and hip girdle muscles — the large muscle groups that encircle your upper and lower body. The hallmark features that set it apart:

Morning stiffness — and lots of it. Stiffness that lasts an hour or more after waking is diagnostic criteria for PMR. This isn't just morning creakiness. Many people describe not being able to raise their arms overhead to wash their hair, or needing help getting up from a chair.

Symmetric involvement. PMR typically affects both shoulders and both hips, usually at the same time — not one side first.

  • Fatty fish (salmon, mackerel, sardines, anchovies) 2–3 times per week
  • Extra virgin olive oil as the primary fat
  • Abundant vegetables, especially dark leafy greens and colorful varieties
  • Legumes — lentils, chickpeas, beans — as primary protein sources
  • Whole grains rather than refined carbohydrates
  • Berries and fruit for antioxidants
  • Moderate nuts, especially walnuts (high in plant-based omega-3s)

Exercise during active PMR needs to be thoughtful — when inflammation is high, pushing hard backfires. But gentle, consistent movement is genuinely important for:

  • Maintaining muscle mass (fighting steroid-related muscle wasting)
  • Supporting bone density
  • Improving morning stiffness and range of motion
  • Mental health and sleep quality

What tends to work:

  • Morning warm-up routines — before getting out of bed, gentle range-of-motion movements for shoulders and hips can reduce the severity of morning stiffness
  • Walking — low-impact, scalable, effective. Even 15–20 minutes daily provides meaningful benefit
  • Gentle yoga or tai chi — specifically targeting range of motion and balance; there's good evidence for both in older adults with inflammatory conditions
  • Swimming or water aerobics — the buoyancy reduces joint load; excellent during moderate disease activity
  • Physical therapy — a PT with experience in inflammatory conditions can develop a personalized routine that accounts for where you are in your steroid taper

Supporting the Taper

The PMR journey is, in many ways, the story of the steroid taper — slowly reducing prednisone dose while watching for symptom return. Flares during tapering are common and frustrating. They don't mean you've failed. They're part of the biology of PMR.

A few things that support a successful taper:

  • Don't rush it — tapering too quickly is the most common cause of flares. Most rheumatologists use a very slow protocol, dropping doses in small increments every 4–6 weeks
  • Track your symptoms carefully — distinguishing true disease flares from steroid withdrawal symptoms (which can mimic PMR) helps you and your doctor make better decisions
  • Support your body holistically — anti-inflammatory nutrition, gentle movement, sleep quality, and stress management all reduce the overall inflammatory load on your system
